SCHOOL SCREENINGS AND YEARLY PHYSICALS

There are many requirements to attend public school in the state of Texas depending on your child's age group and we want to ensure your family is prepared for each school year to set them up for the most success possible.

Please note that if your child is not up to date on their check ups, we will need to complete a well check before filling out any paperwork.

Pre-K & Kindergarten

At your child's 4 year old well check, we will administer the required vaccines for their age and complete their first hearing and vision screenings. Don't forget to ask for a copy of their shot record if needed.

Junior High

Once your child turns 11, they are eligible for a few more vaccines and their school will need an updated copy of their shot record. If they are starting sports, we can also complete a sports' physical in the same appointment.

High School

We highly encourage any child or teenager involved in sports to receive a sports' physical every year in addition to their annual well check. After your child turns 16, they will be in need of additional vaccines as well.

After High School

If your child is still under our care after high school, we can help prepare them for college by administering their Meningitis B vaccine as required in the state of Texas and completing an annual physical.
