Newborn Program and New Patient Care

Newborn Care Program

Our Newborn Care Program is for the new parent that needs a step-by-step guide on how to navigate newborn care before and after your baby has arrived.

We have created the following guidelines so that you can have a smooth and easy transition into care with our awesome Nuby Pediatrics team! Please reach out to our office if you have any questions.

STEP ONE

Call our office if you would like to book a Prenatal Consultation. These visits are not required, but are available for parents that have many questions prior to booking their first newborn appointment. We've gathered our top 5 questions for your convenience!

Prenatal Consult Questions

STEP TWO

After your baby is born, call us and our Newborn Coordinator will walk you through the insurance process and provide instructions on how to fill out newborn paperwork. We will then make your first appointment with one of our providers.

STEP THREE

Please bring a valid ID, your newborn discharge paperwork, and insurance card with you to your first appointment. When you first walk in, check-in at the front desk and complete the electronic survey provided by our staff before seeing the provider.

PLEASE NOTE: A biological parent or legal guardian who has provided proper documentation MUST accompany the patient at their first appointment. After the initial visit, anyone listed on the new patient paperwork may bring the child in for additional appointments, as long as they provide a valid ID when checking in at the front desk.

New Patient Care

Nuby Pediatrics is currently accepting new patients and siblings of established patients. We encourage those looking for a warm, family-centered environment to learn more about our services here and visit our social media pages.

If you are interested in transferring your child's medical care to Nuby Pediatrics, please follow the guidelines below.

STEP ONE

The first step in becoming a new patient is to locate your child's full shot record. Then, call our New Patient Coordinator and let us know you would like to transfer care so we can request medical records from your previous physician.

STEP TWO

The next step in transferring your child's care is to fill out our new patient paperwork. Once these forms are filled out and received by our office staff, our New Patient Coordinator will call you to schedule your first appointment.

STEP THREE

On the day of your child's appointment, please bring a valid ID, insurance card, and a list of current medications. Check in at the front desk and complete any required surveys. Don't forget to press "Submit" before seeing the provider!
